Radiological diagnosis can be difficult at this stage as the intra-sinus soft tissue or mucosal thickening may be minimal and not very obvious, and in addition the bone erosion may be very subtle. As the invasive fungi tend to spread along the blood vessels, the bony walls of the sinuses may appear unremarkable, even when the disease has extended outside the sinus. Bone erosion is a clear sign that the disease is established, but in general this manifestation is only seen in the later stages of the disease [7, 8].

Obliteration and stranding in the peri-antral fat is a subtle sign of such extension and must be looked for in all patients at risk for acute invasive fungal sinusitis [3] (Figs. 2 and 3). Possible theories have been propounded to explain early peri-antral soft tissue infiltration prior to bone changes in invasive fungal sinusitis [3] – propensity of these fungi to initiate thrombosis could result in vascular congestion, in which case, the peri-antral soft tissue infiltration may be that of peri-antral “edema”; alternatively, the peri-antral soft tissue infiltration could represent the presence of fungal elements outside the maxillary sinus, which may be explained by the tendency these fungi have to spread along blood vessels.
